$210 Per Player for Camp (Includes Camp T-Shirt)

(Per health and safety guidelines of Massachusetts youth summer camps) You must provide a Certificate of Immunization for each player you are registering. You will have the opportunity to upload these forms while filling out the google form sign-up below.

NOTE: Please use the "Updated Immunization Form Upload" page under our more tab to upload your player's Immunization form(s), only if the forms have been updated since signing up for the camp. We will use this so that you do not have to go through the entire sign-up process again if you need to resubmit the Immunization Form(s). Thank you
